Children’s Habilitation Center’s provides a service bridge between an acute care hospital & home for infants and children who have high medical needs.

Without question, CHC is a leader in providing total habilitative care for infants and children. The skill and experience of our dedicated staff includes a Board Certified Pediatric Physician, two Advanced Nurse Practitioners, Nurses, Respiratory Therapists, Physical Therapists, Occupational Therapists, Registered Dietician, Certified Teachers and many additional licensed and certified professionals providing quality care.

Children's Habilitation Center is licensed by the Illinois Department of Public Health (IDPH) as a long-term skilled pediatric facility for children under the age of 22.
